This medical referral form is for hypnotherapy clients who may have mental health issues that are contraindicated or are looking for hypnotherapy assistance with a physical issue. This is a letter to send to your doctor or mental health professional to get their official approval. Please remember that I am a certified hypnotherapist and can help you with vocational self development. I am NOT a licensed medical professional. Getting this waiver signed is for both your protection and mine.

If you elected to book a hypnotherapy session without a medical referral, you do not need to submit this form. If you are waiting on medical approval, please note that these forms are due by the time of your session or you will have to reschedule.
